Terms & Conditions
By signing this document, you understand and agree to the following terms and conditions:
- Standard consultations are 50 minutes and a fee of $250 is payable on the day.
- Payments that cannot be paid in full on the day will incur a $15 administration fee.
- All letters, phone calls, documentation and reports outside of scheduled appointments will incur a fee invoiced at the hourly rate.
- All missed appointments or failure to provide 24 hours notice of cancellation will incur a $100 fee. Continual missed appointments and/or short notice cancellations without sufficient cause may jeopardise future services being provided.
- All information provided is kept confidentially and utilised for the purpose of providing best care.
- Camac Clinical Psychology has limited reception services. You may leave a message, text or send an email, and your contact will be returned.
- In cases of emergency please contact either your GP, Lifeline on 13 11 14, your local Mental Health line on 1800 011 511, Beyond Blue, or 000 in a life threatening emergency.
